A consultation form will be emailed to you FROM FACES CONSENT INFO within 24 hours of your appointment for you to complete – IF THIS HAS NOT BEEN COMPLETED BY YOUR APPOINTMENT – THEN YOUR APPOINTMENT WILL BE CANCELLED – No refunds given.
